New Client FormS
New clients are required to complete a new client packet prior to their initial appointment. An email with a link to the on-line forms will be sent to new clients prior to their initial appointment. The new client packet includes privacy practices, client history questionnaire, consent for treatment and services contract, and payment authorization form.
